Salary: Grade 4, £14,926 - £15,689 per annum. £12.18 - £12.80 per hour

23.5 hours per week

Your role:

You will deliver a range of high-quality administrative support within the Home First Independence Team, Urgent Care. You will be supporting within an integrated team of social and health professionals to facilitate discharges from hospital and community settings. You will act as first point of contact for the team. You will be responsive and ensure incoming requests and enquiries by phone and email are dealt with effectively and efficiently. You will access and record information on internal and external systems accurately in a timely manner.

You will liaise with a range of professionals, families and adults, locality operational teams and council departments.

Your day-to-day duties will assist with the achievement of the team’s functions through the delivery of excellent customer care to service users and colleagues in order to achieve the best outcomes and deliver against The Deal 2030 principles.

The service operates 9am-5pm, 7 days a week. You will be required to work flexibly and support a weekend and bank holiday rota, as and when required.

You will be a positive, self-motivated individual who is prepared to work differently and is accountable for the results. You will be able to work independently and as a team member. You will be a great communicator and will actively engage with service users, family members, carers, service providers and other professionals from all sectors of the community and have the interpersonal skills to appropriately challenge when necessary.

You will provide excellent customer care at all times and have the ability to work flexibly, putting the needs of the service first and working as part of a team to deliver an outstanding service.
